When being in hospital you could come out with anything from a new diagnosis or tablet to a new internal organ. And this is something that can be very daunting and cause a lot of confusion to some people.
For patients that have recently been discharged from hospitals, Jaysons pharmacy is here to help.
What can we do
If you live in the Long Eaton or Wollaton area, call in or give us a ring for a chat with our pharmacist, who will be able to help. The pharmacist will have a look at your medications and conduct a medication usage review, where they will look for irregularities, changes in your regime to simplify it and how to most effectively take your tablets i.e. what time, with water or food etc. As part of this review the pharmacist will be able to provide information on your condition and how the medication works and what it does to better improve your understanding of the condition being treated. As well as looking at the medication the pharmacist will be able to provide lifestyle advice to help improve the efficacy of the medication and the quality of life with the condition.
Another useful tool that we use here at Jaysons Pharmacy is compliance aids. These are an effective way of keeping your medication in check and making sure that all your medication is taken at the correct time. When using compliance aid the dispensers in the pharmacy dispense the medications into trays with days of the week and allotted times on each day for taking the medication (Morning, lunch time, tea time, and night time). These are a very helpful tool for patients who have come out of hospital with new drugs and more tablets that people can keep on top of.
As well as providing help to the patients, Jaysons Pharmacy can liaise with the doctors and come up with a plan of action that will help somebody discharged from hospital to lead a normal and happy life with the condition and the medication.
If you are discharged from hospital and have been put on warfarin, Jaysons Pharmacy is here to make your life easier. Transferring over to us is easy. At the hospital you can request to have your warfarin monitored at Jaysons and the hospital will fax your details over to us so we have a copy. Then we will give you a call to arrange your first appointment with us. Or, just call into the pharmacy and we will ask for your records from the hospital. It couldn’t be simpler. What else can we do?
As part of our pick up and drop off service we can order and collect your prescriptions from the surgery and have them delivered to your home or have them waiting for collection in the pharmacy.
